Semen Analysis for Infertility: Why This Simple Test Should Not Be Delayed.

I recently reflected on the fertility journey of a couple I had been seeing for several years. Their story is a reminder of why a Semen Analysis is an essential part of infertility evaluation and why both partners should be assessed early.
A Couple's Five-Year Fertility Journey
Mr. and Mrs. James had been coming to see me for fertility treatment for several years.
They were settled in the UAE and visited Kerala every year during their annual vacation. Usually, they would stay for about a month, and during each visit, they would come to my clinic to continue their fertility treatment.
They first consulted me after struggling to conceive following their marriage.
We began their fertility evaluation.
The female partner underwent the necessary investigations, and we assessed factors such as her menstrual cycles, ovulation and reproductive health. Treatment was planned accordingly.
But every year, I also recommended one important test for the male partner:
A semen analysis.
Mr. James was hesitant. This is something many fertility specialists encounter. Some men feel anxious about undergoing a semen analysis. Some feel embarrassed. Others worry about what the report may reveal.
And so, despite repeated advice, the semen analysis was postponed. For nearly five years.
Why Is Semen Analysis Important in Infertility?
Infertility is not only a woman's problem.
Pregnancy depends on contributions from both partners. For conception to occur, several factors need to work together, including:
Ovulation and egg quality
Healthy fallopian tubes
A receptive uterus
Adequate sperm production
Healthy sperm movement
Appropriate timing of intercourse
This is why infertility should always be approached as a couple's condition.
A semen analysis is one of the most important initial tests for evaluating male fertility. It can provide valuable information about the quality and quantity of sperm and help doctors understand whether male factors may be contributing to difficulty in conceiving.
What Does a Semen Analysis Check?
A semen analysis evaluates several important aspects of male fertility.
1. Sperm Count
Sperm concentration refers to the number of sperm present in the semen.
A lower-than-expected sperm concentration is known as oligozoospermia.
2. Sperm Motility
Motility refers to how well sperm move.
Sperm need to move effectively through the female reproductive tract to reach and fertilise the egg. Reduced sperm movement is known as asthenozoospermia.
3. Sperm Morphology
Morphology refers to the shape and structure of sperm. A reduced proportion of normally shaped sperm is known as teratozoospermia.
4. Other Semen Parameters
Depending on the laboratory and clinical situation, a semen analysis may also provide information about semen volume, total sperm count and other characteristics of the sample.
Together, these findings help doctors understand the male partner's fertility potential and decide whether additional evaluation or treatment may be necessary.
Five Years Later, We Finally Did the Semen Analysis
After several years of fertility treatment, I once again explained to Mr. James why completing the male fertility evaluation was important.
This time, he agreed. The semen analysis was performed.
The results showed:
Reduced sperm count
Reduced sperm motility
Reduced percentage of normal sperm forms
When abnormalities are present in sperm count, movement and morphology, the condition may be described as Oligo-Astheno-Teratozoospermia, often referred to as OAT.
For the first time in their five-year fertility journey, we had a clearer picture of the male factor.
And that changed how we could approach their treatment.
An abnormal semen analysis does not automatically mean that natural conception is impossible.
Semen parameters can be influenced by several factors, including:
Smoking
Excessive alcohol consumption
Obesity
Poor nutrition
Stress
Lack of adequate sleep
Excessive heat exposure
Certain medications
Varicocele
Hormonal problems
Medical conditions
Recent fever or illness
Sperm parameters may also vary between samples. Therefore, one semen analysis should always be interpreted in the context of the person's medical history and the couple's overall fertility evaluation. Depending on the findings, a doctor may recommend repeat testing or further male fertility assessment.
Once We Identified the Problem, We Could Address It
The important thing about a fertility evaluation is simple:
You cannot address a problem that has not been identified.
Once we had the semen analysis results, we could focus on improving the factors that might be contributing to his fertility concerns.
Appropriate medical treatment was advised based on his clinical condition.
We also discussed important lifestyle modifications that could support overall sperm health, including:
Maintaining a healthy weight
Eating a balanced diet
Exercising regularly
Getting adequate sleep
Avoiding smoking
Limiting alcohol
Managing stress
Avoiding excessive heat exposure to the testes
Following medical advice consistently
Male fertility treatment is individualised. What is appropriate for one person may not be appropriate for another.
That is why evaluation by a qualified fertility specialist is important.
The Next Year, They Came Back With Wonderful News
The following year, Mr. and Mrs. James came back to see me during their annual visit. But this time, they had different news.
They had conceived.
After years of waiting, uncertainty and incomplete evaluation, their fertility journey had finally moved forward.
Their story does not mean that every couple with an abnormal semen analysis will conceive simply after taking medication or making lifestyle changes.
Fertility is complex, and every couple's situation is different.
But their journey highlights something extremely important:
Early semen analysis can help prevent unnecessary delays in fertility evaluation and treatment.
